Already receiving care?

Use your established Osmind messaging channel for questions about your care. For scheduling or help finding the right resource, call 1-888-829-2098. Our front desk is open Monday–Friday, 10 a.m.–4 p.m.

Please ask the office for a secure way to send insurance cards, medical records, or completed forms. Do not send these through ordinary website email.

Before your first visit

  1. Contact the office to discuss your needs and visit options.

  2. Confirm your insurance coverage and appointment details.

  3. Complete the intake documents sent by the office. Have your medication list, relevant treatment history, and questions ready.

For immediate danger or a medical emergency, call 911. For suicide or crisis support in the U.S., call or text 988. Website requests and routine messages are not emergency services.

Community care & additional support

These independent services may offer care beyond our practice. Contact each organization directly to confirm services, eligibility, availability, fees, and insurance. Our office can help you discuss which level of care may fit your needs.

Substance-use treatment & recovery

Eating-disorder care

DBT & skills support

Intensive outpatient programs

Electroconvulsive therapy (ECT)

Discuss evaluation and referral options with your clinician. These are external programs.

Neuropsychological & diagnostic assessments

ADHD assessment or neuropsychological evaluation?

An ADHD assessment focuses on symptoms, developmental history, daily functioning, and other possible explanations for difficulties. A neuropsychological evaluation examines a broader range of thinking skills, such as attention, memory, and learning. The scope, length, report, and cost vary by provider and by the questions being evaluated.

If you need documentation for school, work, or an examination, ask the requesting organization what it requires before scheduling testing. A diagnosis alone does not guarantee an accommodation. Learn more about adult ADHD evaluation from NIMH →

Crisis & immediate support

For immediate danger or a medical emergency, call 911. For suicide or emotional crisis support, call or text 988.
