Before sending records, confirm the destination and any patient authorization needed for the disclosure. A referral does not guarantee an appointment or establish care.

Release of information

Ask the front desk for the current release-of-information form and secure completion instructions. The authorization identifies the person or organization, records requested, purpose, and applicable time frame.
